GROWTH BJJ Waiver and Release of Liability
Acknowledgment of Risks
I, the undersigned, acknowledge that I am voluntarily participating in activities at GROWTH BJJ I understand that these activities involve inherent risks, including but not limited to:
- Injuries resulting from equipment use
- Falls and collisions
- Strains and sprains
- Other risks associated with physical exercise
I understand that these risks may result in serious injury or death.
**Release of Liability:**
In consideration of my participation in activities at GROWTH BJJ, I, on behalf of myself, my heirs, executors, and administrators, do hereby release, waive, discharge, and covenant not to sue [Your Gym Name], its owners, employees, and agents from any and all claims, demands, or causes of action arising from any injury or damage to me while participating in activities at GROWTH BJJ.
**Indemnification:**
I agree to indemnify and hold harmless GROWTH BJJ from any loss, liability, damage, or costs, including court costs and attorney’s fees, incurred due to my participation in activities at GROWTH BJJ
**Medical Clearance:**
I confirm that I am in good health and have no physical limitations that would prevent me from participating in the activities at GROWTH BJJ. I have consulted a physician if necessary.
**Photography Release:**
I grant GROWTH BJJ permission to use my image and likeness in any promotional materials, including but not limited to brochures, websites, and social media.
**Acceptance of Terms:**
I have read this waiver and release of liability, and I fully understand its contents. I voluntarily agree to its terms.
